E-Mail, Fax, and Mail Order Form

Use the form below to send your order to us via e-mail, or
click on "Printer Friendly" to get a printable form that you can type in your ordering information, print it,
then fax, or mail your order to Heritage Urns, our contact and mailing information is listed at the bottom of this page. Be sure to include full-payment with your order (check or credit card), we
will process your order, as soon as, the payment method is approved. Make sure your payment includes cost
of the cremation urn and all engraving options, as well as, the shipping cost and taxes (applicable IL orders), as well as, any discounts.
If you are engraving special wording (wording can be faxed, mailed, or emailed to us), a photo, or clip-art make sure
to include it with this order form and your payment, do not fax photos and clip-art. If engraving on front and
back of urn it may take 5 extra days before we can ship urn out. If emailing a digital photograph or clip-art please email it with a minimum of (300 dpi) or preferably (600
dpi or greater). Please type the full name that your order is place
under, in the subject area of your email message and email it to customerservice@heritageurns.net. If you do not have a digital photo, you can send your photo via USPS to address below. Your photos will
then be scanned and returned to you. Please send more than one photo/clip-art and number them by most preferred
as #1, second choice #2, and so on. Due to the labor intensity of this detailed engraving please allow 7-10
days for completion of these urns.

via E-Mail: You need to know exactly what you are ordering, if you leave this page to view information
elsewhere on our site, anything that you typed in will be lost, and you will have to start over. Fill out form below
completely then, simply "click" once on the "Submit My Order via E-Mail" button located at the
bottom of this page, and your order will be submitted via e-mail to us. If you don't "click" on the "Submit My
Order via E-Mail" button you can print a copy of your filled out order below and then fax it or mail
it to us. You should keep a copy for your records.
The "Printer Friendly" way:
Click on the "Printer Friendly" button, located at the bottom of this page. Then type in your
information and print a copy to fax or mail your order to us (we suggest you print a 2nd copy for your
records). You can also just print a blank page of this form and then fill the form out by handwriting in all
the information, and then fax it or mail it to us. Be sure to include your Shipping Method
(or we send it Standard UPS for FREE**) **Alaska and Hawaii orders shipped USPS, FREE Shipping ONLY
available in the U.S.A., Canadian orders are shipped UPS or USPS and will be charged accordingly. We look
for the shipping method that will get your order to you the fastest.
